got all but few medals/achievements

Save 17 (don't have jack's loyalty/didn't use Omega relay)
Save 16 (have everyone's loyalty except for jack's even after their loyalty quest...completed game/all side quests - jack died, didn't get entire crew, used relay...do over from save 17)
Save 10 - 14 (can redo jack's mission for loyalty...then have everyone else's loyalty ...can do over 6 other side quests... and then complete the game over again doing the relay to have full crew and full sqaud)...pick up extra Zaeed in omega to extend squad... and complete his loyalty quest...10 below is to redo grunt's mission... and a few missed upgrades I made...
complete game with everyone's loyalty...to earn that achievement point
close cockpit window during relay...just in case that's a tiny in-game factor...
find more heavy weapon upgrades
etc...
